Once you have changed your name with Social Security, you must complete a Name Change form. You must return the form with your new government-issued ID and supporting documentation confirming the change (marriage certificate, divorce decree etc.). Your personal information can be updated through cudonline under User Options>Change Address, etc. You can also change your personal information by completing an Account Update form or in person at one of our branches.
